Your Employee ID Number is Required for the Tobacco Cessation Program
You need to provide your Employee ID number when you enroll in the tobacco cessation program. To find your Employee ID number, check your electronic pay statement or your Prescription ID card, or request it from your HR representative. Covered spouses should use your Employee ID number + S (for example, 12233S). Other eligible dependents over age 18 do not need to provide an Employee ID number.
